Home Care

Home Care is a person-centered care coordination model that promotes an individual’s ability to live independently through a process that includes the development of a care plan by skilled case managers, tailored to the person’s desires and needs relating to home care.

Home care is the largest service provided by Clermont Senior Services and improves the quality of life of frail older adults by providing assistance with personal care services and light housekeeping.  These services include:

• Assistance with bathing and grooming
• Help with laundry
• Assistance with vacuuming
• Cleaning the bathroom and kitchen

These services provide for a safe and healthy environment, prolonging the individual’s opportunity to remain living as independently as possible for as long as possible.

Eligibility

• Age 60 and over
• Resident of Clermont County
• Unable to perform all or some of the activities of daily living
• Has little or no assistance in the home from family or friends
• Assessment by a social worker or nurse
• Additional eligibility requirements may apply for each program
• Some customers may qualify for financial assistance through the PASSPORT program
